September 7 Begins our New Program Year
Mark your calendar and be a regular attendee of Grand Rounds. For our new physicians, Grand Rounds are scheduled 8:00 a.m., Wednesday at St. Luke's RMC in the Anderson Center, 12:30 p.m. at Mercy Medical Center in the Winter Room, 8:00 a.m., Thursday at the VA Medical Center in the library conference room and 8:00 a.m., Friday at Saint Alphonsus in the Centennial Room. Grand Rounds are planned for the primary care provider but will be useful to virtually any specialist. All Grand Rounds provide one hour of Category I CME credit and your attending will automatically be added to your individual credit report. This year's first presenter will be Margaret Doucette, D.O. addressing "The Advances in Wound Healing and Hyperbaric Medicine". We hope you'll make ACMEC a part of your routine in the year ahead.
2005 Needs Assessment Results
Response to this year's needs assessment survey have exceeded expectations. ACMEC has received several hundred requests for topics. A list of topics that is greatly expanding and indicative of the explosion of medical information.Top requests continue to be obesity, diabetes, HTN, pain control, and psychopharmacology. There was a trend toward pediatrics in the request, from neonatology to the adolescent workup. Another trend seemed to focus on geriatric issues with numerous requests for talks on dementia, stroke management, and heart failure.
We asked you for feedback on how much an hour of CME is worth and how you view bias in our Grand Rounds presentations. We were pleased by 89% responding that the programming was balanced. We will need to continue to work towards relieving bias and recognize that it is an inherent issue in using industry support.
You also told us (61%) that $10-30/hr credit was about right for an hour of CME. As we struggle with financing CME into this new ERA, we will take your input into our decision making process.

New Digs at Saint Alphonsus RMC
Beginning September 9, our weekly Grand Rounds at Saint Alphonsus RMC will move to a new structure next to the Liberty building. Parking is readily available. The Liberty building is directly behind the main Saint Alphonsus campus off of Liberty Street. The new structure will retain the designation of the Centennial Suite. Call us if you need directions.

A couple of redneck hunters are out in the woods when one of them falls to the ground. He doesn't seem to be breathing and his eyes are rolled back in his head. The other redneck starts to panic, then whips out his cell phone and calls 911. He frantically blurts out to the operator, "O my gawd! Help! My friend just died. He's dead! What can I do?" The operator, trying to calm him says, "Take it easy. I can help. Just listen to me and follow my instructions. First, let's make sure he's dead." There's a short pause, and the operator hears a loud gun shot!!! The redneck comes back on the line and says, "OK, now what?"

Years ago Nancy Reagan, President Reagan's wife, was criticized for her campaign against drug use in teens entitled "Just Say No". It was felt to be too simplistic but saying no is often an appropriate response. Any two year old has the word mastered. Our profession though has difficulty saying no.
We physicians need to say "no" more often.
No, to inappropriate prescribing and drug companies who shower our offices with gifts, favors, and samples to influence our prescribing habits.
No, to inappropriate tests or procedures which do not replace a thorough history and physical.
No, to insurance companies who make decisions which interfere with the doctor/patient relationship.
No, to health plans who would have you pay attention to their bottom line without regard to patient well-being.
No, to patients who ask for certain medications and tests which you deem inappropriate.
No, to patients who ask for extended disability or time off, because their insurance covers it.
No, to self-treatment for your self and your family.
No, to patients who are seeking habit forming medicine inappropriately.
It is often difficult to say no to patients because it may lead to conflict and each of us wants to be liked by our patients. Many times, though it may be the appropriate thing to do. Saying no in a kind manner with explanation may be the best medicine in certain circumstances. Medicine has a long tradition of doing the right thing for patients and many times saying no is the right thing.
